We require all clients to complete and sign our consent form before their appointment. We also request that you attach a legal form of ID and the consent form (use the form below to submit your consent form and ID). This can be done either ahead of time or during your appointment. Your privacy is important to us, and we will keep all forms and information confidential.

PLEASE FILL OUT AND INITIAL IN THE BOXES PROVIDED AFTER READING TO SHOW THAT YOU UNDERSTAND EACH PROVISION. FEEL FREE TO ASK ANY QUESTIONS REGARDING THIS WAIVER.

I understand the risks of tattooing, including infection, scarring, allergy to pigment/latex/soap, and melanoma detection challenges. I accept all known and unknown risks and choose to proceed with the tattoo procedure.

I fully release the Tattoo Studio and its associates from any liability, including personal injury or other claims, resulting from my tattoo procedure, whether due to negligence or any other cause. This includes direct and consequential damages.

The Tattoo Studio has given me the full opportunity to ask any question about the procedure and application of my tattoo and all of my questions, if any, have been answered to my total satisfaction.

The Tattoo Studio has given me instructions on the care of my tattoo while it’s healing. I understand and will follow them. I acknowledge that it is possible that the tattoo can become infected, remarkably, if I do not follow the instructions given to me. If any touch-up work to the tattoo is needed due to my own negligence, I agree that the work will be done at my own expense.

I am not under the influence of alcohol or drugs and voluntarily submit to be tattooed by the Tattoo Studio without duress or coercion.

I don't have diabetes, epilepsy, hemophilia, heart issues, or take blood thinners. I have no medical/skin conditions affecting tattooing or healing. If I've had an organ/bone marrow transplant, I've taken necessary antibiotics. I'm not pregnant, nursing, or mentally impaired affecting my tattoo decision.

The Tattoo Studio is not responsible for the meaning or spelling of the symbol or text I provided or chose from the flash (design) sheets.

Variations in color and design may exist between the tattoo art I selected and the actual tattoo when applied to my body. I also understand that over time, my tattoo's colors and clarity will fade due to unprotected exposure to the sun and the naturally occurring dispersion of pigment under the skin.

A tattoo is a permanent change to my appearance. It can only be removed by laser or surgical means, which can be disfiguring and /or costly and will, in all likelihood, not restore my skin to its exact appearance before being tattooed.

I release the right to any photographs taken of me and the tattoo and give consent in advance to their reproduction in print or electronic form. (For assurance, if you do not initial this provision, please inform the Tattoo Studio NOT to take any pictures of you and your completed tattoo).

I agree that the Tattoo Studio has a NO REFUND policy on deposits, tattoos, and/or retail sales, and I will not ask for a refund for any reason whatsoever.

I'll cover the Tattoo Studio's attorney fees if they prevail in any legal action I initiate. I agree that Fairfax County, Virginia courts have exclusive jurisdiction over any disputes related to this agreement.

I've had ample time to read and understand this agreement and acknowledge it's a legal contract. If any part of this release is invalid, the rest remains effective. I confirm I am of legal age, have provided valid ID, and am competent to sign.
